Benefit

Eligibility

Health Insurance: Choice of three health plan options, each with choice of full Cigna OAP network or narrow LocalPlus network. Options provide varied levels of cost sharing. Coverage available for spouses & children. 

 Option A: traditional health plan

 Option B & C: high deductible health plan with an HSA

Standard benefit eligibility:  All regular full-time & part-time employees budgeted to work 40 or more hours per pay period, coverage is effective the 1st of the month following 30 days of employment.

Dental Insurance:  Choice of PPO & DHMO plan options through Delta Dental of Illinois. Coverage available for spouses & children.

Vision Benefit: Two vision plan options through United Heatlhcare. Coverage available for spouses and children.

Standard benefit eligibility.

Health Savings Account: Pre-tax savings account to cover eligible medical, dental & vision expenses.  Employer contribution based on employee/family coverage tier.

Limited to employees enrolled in a high deductible health plan. (Must meet IRS eligibility guidelines).

Health Care Flexible Spending Accounts:  Pre-tax spending account to cover eligible medical, dental & vision expenses. 

Standard benefit eligibility.

Retirement Savings Plan 403(b):  All employees eligible to contribute to pre-tax retirement savings plan.  Hospital will match contributions up to a maximum of 5% of eligible earnings. Vested in hospital matching contributions after three years of service.

All employees are eligible to participate at time of hire.

Employee Life Insurance:  Hospital paid basic life insurance of one times annual base salary.  Employee paid coverage available up to a total of three times annual base salary.

Standard benefit eligibility.

Dependent Life Insurance:  Employee paid coverage available for spouse and children.

Accidental Death & Dismemberment:  Employee paid coverage available for employee, spouse and children.

Paid Time Off (PTO):  24 days accrued per year for new employees to be used for holidays (7), vacation, personal illness (in conjunction with short-term disability) & other eligible time off.  PTO accrued based on hours worked, increased accrual based on years of service. 

All regular full-time & part-time employees budgeted to work 40 or more hours per pay period are eligible to utilize the benefit after 90 days of employment. (Holidays only during first 90 days). PTO accrual rate based on date of hire and prorated based on budgeted FTE status.

Short-Term Disability:  Hospital paid coverage.  60% weekly pay replacement for a maximum of 26 weeks.

All regular full-time & part-time employees budgeted to work 40 or more hours per pay period. Coverage effective after six months of benefit eligibility.

Long-Term Disability: Hospital paid coverage equal to 60% of monthly pay replacement. Option to purchase additional coverage of 66 2/3% pay replacement.

All regular full-time & part-time employees budgeted to work 40 or more hours per pay period. Coverage effective after 1 year of benefit eligibility.

Employee Assistance Program (EAP): Available for employee, dependents & extended family, provides confidential access to a variety of resources to help deal with personal issues.

Wellness Programs:  A variety of resources and tools are available to support & reward healthy activities & behaviors.  Access to condition management programs, health coaching & education and healthy competitions.

Dependent Care Flexible Spending Account:  Pre-tax employee spending account to cover eligible dependent child &/or adult care expenses. 

Standard benefit eligibility.

Adoption Assistance: Reimbursement of $5,000 per child to help offset eligible adoption expenses.

All regular employees budgeted to work 40 or more hours per pay period. Eligible after six months of employment.

Bright Start 529 College Savings Program: Employees can contribute to a college savings account through direct deposit. (State of Illinois program)

All employees are eligible.

Tuition Assistance: Up to $5,000 of monetary assistance per year for your professional development. (Pro-rated for part-time employees).

Pre-Tax Transit Benefit Option: Pre-Tax commuter benefit option available to cover public transportation and parking costs for commuting to/from work.

Parking:  Parking arrangements available in various garages.  Availability may be limited.

All employees are eligible.

Shuttle Service: Available from all major train stations for a discounted fee.

All employees are eligible.

Healthcare Associates Credit Union: Offers checking & saving accounts, loans & credit cards.

All employees are eligible.

Voluntary Benefit Programs:  Employees can purchase a number of discounted, portable insurance products (auto, home, health, pet, long term care etc.)

All employees are eligible.

Hospital Discount: 50% discount off Lurie Children’s hospital charges after insurance has been applied.

All employee dependent children are eligible.

Other Discounts: Restaurants, hotels, cell phones, computers, amusement parks, wedding invitations, movie tickets, etc.

Employee Referral Program:  Reward program for referring applicants who are hired to work at Lurie Children’s.

All employees are eligible.
