If you are extended an offer for employment, there are several employment requirements which must be completed prior to your first day of work. Employment is contingent upon successful completion of the Corporate Health screening and completion of all other employment requirements.
Proof of immunity to Measles/Mumps/Rubella (MMR)- AND -
Proof of immunity to Varicella (Chicken Pox)
Documentation of Tuberculosis (TB) tests
Proof of influenza (flu) vaccination
Proof of adult Tetanus/Diphtheria/Pertusis (Tdap) vaccination
Proof of immunity to Hepatitis B
If you do not have these health records in your possession, you may be able to get copies from the following sources:your doctor, high school/college (if a recent student), or current/former employer.

Verification of Licensure, Registration, and/or Certification
