Employment Requirements

If you are extended an offer for employment, there are several employment requirements which must be completed prior to your first day of work. Employment is contingent upon successful completion of the Corporate Health screening and completion of all other employment requirements.

Health/Immunization/Blood titers

  • Proof of immunity to Measles/Mumps/Rubella (MMR)  - AND -
  • Proof of immunity to Varicella (Chicken Pox)             
  • Documentation of Tuberculosis (TB) tests
  • Proof of influenza (flu) vaccination 
  • Proof of adult Tetanus/Diphtheria/Pertusis (Tdap) vaccination
  • Proof of immunity to Hepatitis B

If you do not have these health records in your possession, you may be able to get copies from the following sources:  your doctor, high school/college (if a recent student), or current/former employer.

 

Drug Screen 

 

Documentation of Eligibility To Work in the US  

 

Verification of Licensure, Registration, and/or Certification

This is required for all positions which require a license, registration, or certification 

 

Education Verification   

 

Criminal Background Check
